1095-C tax forms for 2023 with health insurance coverage information offered to you by Albertsons will be available to download starting February 12 or mailed to your home on February 16. This form includes information about the health insurance coverage offered to you by Albertsons Companies. This form is required to submit your tax return if you live in California. This form is not required for your tax return if you live in a different state.

If you elected to receive your 1095-C form electronically in the past, you can access your 2023 tax form at www.mytaxform.com starting February 12. You will receive an email on February 12 with the subject line IMPORTANT TAX RETURN DOCUMENT AVAILABLE from noreply@TaxFormManagement.com letting you know your 2023 tax form is available to download.

If you would like to access your 1095-C tax form electronically but have not previously registered online, you can register at www.mytaxform.com to access a digital copy. To register online, follow these steps:

  1. For Employer Name/Code use 11202 then click the Login button.
  2. In the Register Now box, click on Click here to register.
  3. Follow the prompts and fill in the information requested and click the Continue button to proceed to the next screen.

Please note: Once you are registered, you must log out, then log in again to see your 2023 form.

If you have not elected to receive your 1095-C tax form electronically, a paper copy will be mailed to your home on February 16. If you do not receive your 1095-C tax form by March 1, you can request a duplicate be sent by contacting the Associate Experience Center at 888-255-2269, option 1.
